School Club Information Sheet - Part 1

To be completed by Team Leader/Administrator

Please fill out one form for each club.

The information you're about to submit is very important. It will be used for . . .

  • Obtaining insurance certificates for in-person clubs
  • Applying for facility use permits with individual school boards for in-person clubs
  • Listing your club meeting information on the website
  • Developing Student Registration Forms and Flyers
  • Ensure volunteers meet the training and screening requirements prior to teaching/helping

We encourage you to submit this form at least 2-3 weeks before your club's Back To School night or the first opportunity that you could distribute flyers or start your recruitment process.

Please be considerate and try to refrain from submitting everything on the last minute, ie. the week before your club's start date. Your consideration and cooperation is much appreciated.

Reminder: Once your team members have completed their Worker's Compliance Agreement and Fingerprinting and all the other required screening requirements, make sure you also complete the School Club Information Sheet Part 2 (Personnel Info).
