5-Day Club Supervisors Instructions

Dear CYIA™ SUPERVISORS,

Thank you for sacrificing your time and effort for this year’s CYIA students.

First and foremost, please sign a WCA Form (annually) and ensure your Background Check/LiveScan (once every 5 years) records are still valid.

Here is the checklist once you’re given location/date/time for your team:

Day 1

• You must contact all your CYIAers to be present at the host club 1-HOUR prior to the start of club time.
• Bring your Daily Assignment Chart with you.
• Ensure all supplies are prepared for the team (for games, etc.) Reimbursement is available upon request.
• If the team will have themes (crazy hair, funny socks, etc...) each day and/or wet day on Day 5, make sure Team #1 communicate to the Host and the children each day
• Keep the supply box with you to bring each day of club
• Team Member #1 is responsible to start the club on-time and end the club on-time.
• Be sure to pray together at the start and end of the club each day
• Make sure all CYIAers are in their proper attires, ie. CYIA shirts.
• Actively observe each team's role from beginning to end, take notes on each team's presentation (do they look like they prepared, not organize, etc..)
• Share your encouragement and observations of the teamwork.
• At the end of the club, use 15 minutes to review and reflect on the day, such as any concerns or comments on how to improve for next day.
• Keep good record of attendance, profession of faith and missions fund for stat reporting purpose.
• Inform the Host a copy of the registration/attendance sheet will be collected at the end of Day 5.
• Remind Team Member #1 to share any profession of faith with the 5DC Host.
• Communicate to Host that during Missionary time, Missions money are collected and children will be encouraged to give to CEF's Mission's Focus and the amount raised will be collected at the end of the 5-DC.
• Make sure to have ALL team members clean up before leaving Host facility

Day 2, 3 & 4 (In addition to Day 1’s Checklist)
• Team must be present at the club 30-MINUTES prior to the start of club time

Day 5 (In addition to Day 1’s Checklist)
• Get a copy of the final registration attendance sheet to 5DC Coordinator Merci Rusli and submit final stat report online.
• Missions money is to be counted and given to 5DC Coordinator, Merci Rusli
